In-Home & On-Site Support Services

Passageway provides in-home and on-site support to individuals who are transitioning from residential treatment to independent living or who are experiencing challenges maintaining stable community housing. Services focus on strengthening the practical skills and routines needed to sustain a safe, healthy, and stable living environment. Staff provide support with household organization, daily living skills, problem-solving, communication, conflict resolution, and connection to needed community resources. The goal is to address challenges early, strengthen independence and housing stability, and help individuals successfully maintain their homes and continue their progress in the community.

Case Management Services

Our case management services help individuals identify their needs, develop individualized goals, and connect with the services and community resources that support recovery and independence. Case managers coordinate services across providers and systems, monitor progress, and advocate for supports that respond to each person’s changing needs and goals. Our goal is to strengthen the connections and skills needed for meaningful, self-directed lives and safe community reintegration.

Psychosocial Rehabilitation Services

Passageway’s Psychosocial Rehabilitation program provides structured, therapeutic, and educational group services designed to reduce disability and strengthen the skills needed to live more independently and successfully in the community. Participants develop and practice skills in activities of daily living, self-care, communication, emotional regulation, problem-solving, social interaction, and responsible decision-making.

Through a multidisciplinary approach, the program helps individuals improve daily functioning, develop positive and prosocial behaviors, build healthy relationships, and increase their ability to manage the demands of community living, education, and employment. These services support greater independence and successful community integration while reinforcing safety, responsibility, and recovery.

Individual and Group Therapy Services

Passageway provides individual and group therapy to support recovery, improve emotional and behavioral functioning, and help individuals develop the skills needed to manage mental health challenges and navigate daily life successfully. Individual therapy provides a confidential setting to address personal needs, strengthen coping and problem-solving skills, develop insight, and work toward greater emotional stability and independence.

Group therapy provides opportunities to practice communication, interpersonal, and emotional regulation skills in a supportive environment while learning from and contributing to the experiences of others. Group topics may address symptom management, coping strategies, relationships, social functioning, relapse prevention, trauma, and other areas that support recovery and successful community living. Together, individual and group therapy promote healthier decision-making, positive relationships, personal responsibility, and safe and sustained community integration.

Psychiatric Consultation Services

Passageway provides psychiatric evaluation and medication management services to support the mental health, stability, and recovery of those we serve. Our psychiatric providers assess each individual’s mental health needs, monitor symptoms and treatment response, and prescribe and adjust medications when clinically indicated. Services include ongoing evaluation of medication effectiveness and side effects, coordination with the treatment team, and education to promote understanding of medications and active participation in treatment. Our approach emphasizes clinical safety, continuity of care, and collaboration to support stability, functional improvement, and successful community living.

Residential Treatment

Passageway provides structured, 24-hour residential treatment and rehabilitation services in a supportive, homelike environment for individuals working toward greater independence and community reintegration. As a Residential Level II facility, Passageway provides ongoing supervision, support, and assistance to individuals with varying levels of independent living skills, while promoting recovery, self-determination, personal responsibility, and meaningful participation in daily life. Our multidisciplinary team includes nursing support to address identified health and medication-related needs and to promote continuity of care.

Independent Living & Community Housing (Outreach)

Passageway offers individuals completing residential treatment an opportunity to transition to greater independence through shared community-based housing in Passageway-owned residences. Individuals share living spaces with carefully considered housemates, with placements based on individual preferences, compatibility, and housing availability. Housing is offered at an affordable rate designed to remain accessible to individuals living on SSI or other limited income.

Residents continue to receive support from Passageway as they build the skills and stability needed for successful independent living. Participation in case management, psychosocial rehabilitation, and psychiatric and medication management services is required as part of the program, providing ongoing support with daily functioning, wellness, community living, and recovery. This step-down model allows individuals to experience greater independence while maintaining the structure and supports that promote stability, accountability, and safe, sustained community integration.
